Fatty Liver Grade 1, 2 and 3: Meaning, Symptoms and Treatment

What “fatty liver grade 1, 2 or 3” on your ultrasound means, whether it can be reversed, and when to see a liver specialist.

Fatty Liver Grade 1, 2 and 3: Meaning, Symptoms and Treatment

“Fatty liver” is one of the most common findings on an abdominal ultrasound in India. It means fat has built up inside liver cells. For most people it is reversible — but if ignored, it can slowly progress to inflammation, scarring (fibrosis) and cirrhosis.

What do fatty liver grades mean?

  • Grade 1 (mild): a small amount of fat. Usually no symptoms and very reversible with lifestyle change.
  • Grade 2 (moderate): more fat; liver enzymes may be raised. Needs active lifestyle change and medical follow-up.
  • Grade 3 (severe): a large amount of fat. Higher chance of inflammation and scarring — see a specialist.

Important: ultrasound grade shows the amount of fat, not the amount of scarring. A FibroScan or blood-based scores are used to check for fibrosis.

Symptoms of fatty liver

Most people have no symptoms. Some notice tiredness, heaviness in the upper right abdomen or indigestion. Swelling of the abdomen, yellow eyes or vomiting blood are signs of advanced disease and need urgent attention.

Causes and risk factors

  • Excess weight, especially around the waist
  • Diabetes, high cholesterol and high blood pressure
  • Alcohol
  • Inactive lifestyle and diets high in sugar, refined flour and fried food

Can fatty liver be reversed? How long does it take?

Yes, in most cases. Losing about 7–10% of body weight steadily can reduce liver fat significantly. Grade 1 often improves within a few months of consistent change; higher grades take longer and need monitoring.

What to eat with fatty liver

  • Cut down sugar, sweets, soft drinks, maida and fried food
  • Choose whole grains, dal, vegetables, salads and fruits in sensible portions
  • Avoid alcohol completely
  • Walk or exercise at least 30 minutes a day, five days a week

When to see a liver specialist

  • Grade 2 or 3 fatty liver, or raised liver enzymes
  • Diabetes or obesity with fatty liver
  • A FibroScan showing fibrosis
  • Any sign of advanced disease — read about liver cirrhosis
